☑︎ No refund will be issued once my cosmetic treatment has commenced.

☑︎ I consent to before and after photographic documentation for FLOUNOU ARTISTRY records and promotional purposes.

☑︎ Failure to disclose all medical conditions or medications may impact results and safety.

☑︎ I agree to annual or periodic touch-up appointments as required to maintain my cosmetic tattoo.
